How to View Allergy Information

If you are already collecting allergy information, there are several ways to view this data:

    • In the Medical Information section of the Person Detail page
    • On the Custom Online Form used to collect allergy information
    • Using a Custom Report which includes the allergy information

Allergy information is considered Medical Information, so access to it is restricted to Admin Users with Medical permissions. The only exception to this is in Custom Reporting or during an emergency override by another admin user.

Viewing Allergy Information on the Person Detail

If you wish to view a specific individual's allergy information, this is the approach to use:

    1. Open the person's account.
    2. Click on the individual's name.
    3. Click Medical Information under Common Tasks.

In the pop-up window, you will see the person's allergy information if two criteria are met:

  Note

If you don't have medical information permissions, you will see the warning message below. In case of an emergency, you can bypass this restriction by clicking the Emergency Access button. UltraCamp will log your information, including user ID and the time of your access. It will also notify Master Administrators of the access.

Viewing Allergy Information on a Custom Online Form

If you wish to view allergy information in the context of a Health Form or other online form, there are two approaches:

    • View an individual form on the Person Detail.
    • View multiple forms using the Online Forms report.

These approaches work well if you are not using Form Excerpts to connect your allergy information or if you want more context.

Viewing an Individual Form

To view allergy information on an individual form, take the following steps:

    1. Open the person's account.
    2. Click on the individual's name.
    3. Expand the Forms section.
    4. Click the image.png print icon to the right of the appropriate form.

Depending on your browser settings, the online form may open in a new tab or window, or it may download to your device.

Viewing Multiple Forms

To view allergy information on multiple forms, take the following steps:

    1. In the Reports menu, expand the Forms section.
    2. Click Online Forms.
    3. From the dropdown menu, Select the Form you wish to view.
    4. [Optional] Use the Who is also in dropdown to choose a stored group to focus your report.
    5. Click Search.
    6. At the bottom of the list, click Print Selected.

UltraCamp will generate a file with all the selected forms in one of three ways:

    • A file will appear in a new window or tab.
    • A file will be downloaded to your device.
    • A job will be started in My Jobs. When complete, you can access the pdf by clicking the Adobe Acrobat Icon next to the job.

You can then access the file and review all the forms in it.

Using a Custom Report to View Allergy Information

Having a Custom Report to reference Allergy information is quick and convenient. It also bypasses the normal Medical Information restrictions.

Once a custom report has been created, you can reference it at any time in the Reports menu in the Custom Reports section.

To create a custom report to view Allergy Information, here are the recommended steps:

    1. In the Reports menu, click Create a custom report.
    2. In the Data Fields tab, add the following items to the workspace from Individual Information:
        • idPerson
        • First Name
        • Last Name
    3. In the Data Fields tab, expand the section for the custom form that contains allergy information.
    4. Add your allergy information question(s) to the workspace.
    5. In the Filters tab, add a Predone Filter and choose the Prompt for Sessions option.
    6. Click the Save Report button.
    7. Give the report a Name and click the Save button.

You can now exit the Report Builder. The report you have created will be found in the Reports menu under the Custom Reports sections, and you run it at any time for up-to-date information.

You can learn more about building Custom Reports here.
